Now that we have learned some ethical issues that must be considered when designing a study, let's put it all together into preparing a consent form.

Presented below is a sample consent form. Look it over and identify all of the necessary elements of consent. (The answers are on the following page)
Project Description


PERSONAL RELATIONSHIPS IN ADULTHOOD


Information is being gathered as part of an introductory class in statistics at Local University. We are asking all students in the class who complete this questionnaire to discuss their beliefs and feelings about themselves and their relationships with others. While there may be no direct benefit to you from participating in this project, we hope to provide students in the class with a learning experience in which the data that they analyze is interesting to them and reflects an established area of study in the field of psychology. We will enter the information gathered anonymously into a database that students will use in the class and a computer laboratory.

We are asking you to answer questions about how you see yourself and your relationships with others. We are also gathering social background data. It should take about 20-25 minutes to answer all of the questions on the survey form. When you are finished, please return the questionnaire to the designated envelope in your computer lab.

There are no known problems or risks with these types of surveys. Some people, though, find it difficult, at times, to think about personal matters. You can decide not to answer specific questions. You also can stop filling out the questions at any time. You can choose not to participate in the project without any consequences for your class grade.

If you find that the questions are stressful or you wish to discuss any part of the project, you may telephone Dr. Nice Teacher, the course instructor, on campus, at 555-6753.

Your answers to questions will be strictly confidential. Your name will not appear anywhere on the questionnaire. Your answers will be used only for teaching purposes and will be presented to the class as part of group statistics.

If you have any questions about the project, feel free to contact Dr. Teacher. Because the questionnaire will be completely anonymous, filling out the questionnaire will be considered consent to participate in the project. Please keep this form for your personal records.
